Upon final payment, send a legible copy of your reconciliation packet. The packet must includes a copy of your travel voucher summary, claim and airfare cost.
- You must send your travel settlement voucher (AKA travel voucher summary) to the Mediterranean DSO Travel Section.
The settlement voucher may consist of two to three pages. You can get your
travel voucher summary on-line. It is the same web site where you can
view/print your leave and earning statements.
- 1) Get access to the web site by clicking on “Need a new PIN.” You will receive a PIN via postal system.
- 2) Enter your SSN “Account Access” and your PIN.
- 3) Select“Travel Voucher Advice of Payment (AOP)."
- 4) Select one of the following: Last 30, 60, 90, or 180 days.
- 5) Select the correct voucher (the travel order listed may not be exactly what is listed on your travel orders).
- 6) Print your travel voucher summary.
- If the Aviano or Incirlik finance offices service you, then you must request a copy directly from the finance office. Please ensure that you are using the latest travel claim form, DD Form 1351-2; version July 2004. This form has a field for your email address.
- It is very important for the Mediterranean DSO Travel Section to
have a copy of your SATO issued tickets, i.e. airline or train tickets. Attach the copy of your travel tickets (costs) to your travel voucher summary
before sending it to the Mediterranean DSO Travel Section.
- The EVT reconciliation procedures for SATO issued tickets (using funded travel orders) only require that you submit a copy of your ticket or receipt showing the cost of the airfare.
- Your travel reconciliation packet should resemble the following:
- Travel voucher summary
- Tickets (airfare and rail tickets copies or receipts showing dollar
cost)
- Travel voucher or subvoucher (a.k.a. claim; DD Form 1351-2 with your signature and principal's signature; without receipts)
- You can fax or email your travel reconciliation packet.
